Position: Outpatient Therapist (Adult)

Credential/Degree: Master’s in Human Services; Professional Licensure preferred; or bachelor’s in a Mental Health field and currently enrolled in a Master’s program on a professional counseling track.

Principle duties, includes, but not limited to:

  • Therapist Service Delivery:
    • Screening
    • Orientation
    • Intake
    • Assessment
    • Treatment planning
    • Counseling
    • Crisis Intervention
    • Client Education
    • Referral
    • Advocate for client rights
    • Monitoring
    • Consultation
    • Reports and Record Keeping
    • Immediate Reporting
    • Crisis Intervention
    • Incident reports
    • Client records and documentation following agency policies
  • General client services:
    • Provide program information to referred clients and permitted family/guardian/others, as well as current clients and permitted family/guardian/others.
    • Initiation, coordination, implementation, and monitoring of the treatment plan.
    • Assist clients to identify treatment needs.
    • Update the treatment plan:
      • No less than quarterly, and
      • As changes occur, and
      • As treatment needs are identified, and
      • As treatment needs are completed.
    • Ensure other service providers, as applicable, are aware of client’s progress or lack of progress in accomplishing identified goals of the treatment plan.
    • Assist clients in obtaining a needed service outside of those available at Adanta using referrals and information.
    • Furnish clients with listings of available providers (as applicable) in the service area.
    • Assist clients in planning resource use and assuring protection of those resources
    • Monitor the health, safety, and welfare of clients.
    • Orient new clients to programs for which they have been referred, as applicable.
    • Provide supports to clients as needed.
  • Maintain an active awareness of the client’s personal and family situations and provide support as needed.
  • Document request for services that cannot be provided and reason that services cannot be provided.
  • Ensure adequate supervision is/ supports are provided based on the client’s identified needs at chosen service
  • Primary responsibility to assist client and permitted family/guardian/others to implement the following:
    • Treatment Plan;
    • Other information or plans to ensure health, safety, and welfare of clients.
    • Respond to and assist client and permitted family/guardian/others requesting clarification on items above.
    • Work with and actively communicate with client and permitted family/guardian/others.
    • Work with and actively communicate with other Adanta staff and other agencies (as permitted) to ensure client receives identified services according to the treatment plan, and to communicate information to ensure the health, safety, and welfare of clients.
    • Work with and actively communicate with client and permitted family/guardian/others to ensure information is shared regarding hospital admissions, hospital discharges, emergency room visits, changes in medications, allegations of abuse/neglect, changes in psychiatric or medical care and assist with the implementation of any changes as instructed to ensure health, safety, and welfare of clients.

Client and Permitted Family/Guardian/Other Services

  • Develop a working relationship with the client and permitted family/guardian/others and encourage participation in the planning and implementation of services.
  • Assist client and permitted family/guardian/others in finding and using services and supportive networks.
  • Assist with interpreting evaluations, reports, and other professional documents for clients and permitted family/guardian/others.
  • Assist with future planning for the client.

Service Requirements

  • 60% direct billable time.
  • Timely completion of reports, service documents, medical record documentation.

Audit Requirements

  • Clean billing audit for each month reviewed. (Less than 4% overall error rate/month.)
  • Zero no notes for each month reviewed.

Agency Responsibilities

  • Actively work together and communicate with staff in all programs to ensure adherence to agency policies and regulations related to the health, safety, and welfare of clients.
  • Complete and submit administrative documents according to agency policies, i.e., timesheet, travel sheet, leave requests, etc.
  • Develop, maintain communication, and serve as liaison with community agencies to assure client’s needs are met.
  • Comply with agency policies and regulations related to service delivery and agency operations.
  • Attend and actively participate in required in-service and out of agency training, workshops, staff meetings, conferences. Implement acquired information as applicable.
  • Actively communicate and share information with other service providers as applicable to ensure client’s health, safety, and welfare at all times.
  • Assist other staff as needed to ensure scheduled service delivery is provided timely and correctly.
  • Encourage a team approach with other staff to accomplish goals/objectives of the agency.
  • Maintain professional image in self and office environment.
  • Facilitate meetings to maintain communication among and between program staff.
  • Assist in performance improvement initiatives and monitoring.
  • Perform any other duties as deemed necessary by management.

SKILL REQUIRED AND DEMONSTRATION OF COMPETENCIES WITHIN REQUIRED TIMEFRAMES

  1. Skill in making clinical impressions using DSM IV.
  2. Completion of New Employee Orientation and Site Orientation.
  3. Completion of required training for certification/licensure and E-Learning.
  4. Successful client engagement as observed by supervisor.
  5. Communication skills appropriate to working with clients and staff.
  6. Specialized training as necessary.

EQUIPMENT NECESSARY TO PERFORM ESSENTIAL FUNCTIONS OF JOB

  1. Working environment includes normal clinic environment, good lighting and ventilation in the work area.
  2. Equipment used to perform essential functions of the job: telephone, copier, fax, adding machine, desk, chair and other related equipment as the job might dictate.
  3. Safety hazards involved in this would be minimal if normal safety practices are followed.

 WORKING CONDITIONS:  Much of work can be performed in an office environment or community setting; must be able to maintain required caseloads and complete necessary Medicaid/Medicare billing documentation; work conditions may be stressful and fast-paced and require a person who is able to prioritize and organize their day in order to meet minimum billing requirements.

PHYSICAL REQUIREMENTS / JOB HAZARDS

  1. Current TB skin test indicating no evidence of communicable disease.
  2. Possible contact with bodily fluids.
  3. Possible physical management (lifting, balancing, maneuvering).
  4. Walking on a regular basis.
  5. Standing on a regular basis.
  6. Sitting on a regular basis.
  7. Driving on an occasional basis.

EFFORT:  Must be able to understand and communicate with people from all socio-economic settings. One must be able to use computers and other office equipment as necessary to complete daily tasks (notes, submitting tickets, completing documentation necessary for billing purposes, etc.). This position requires a person to have knowledge of computer programs such as word, EMRs, and be accurate in data entry/note taking skills; these essential functions require a person to use manual dexterity and fine motor functions throughout one’s daily activities.  One will be frequently sitting, standing, walking and stooping. One must have reliable transportation, maintain a clean driving record, and maintain a driver’s license and automobile insurance throughout employment.

Must be able to defuse potentially dangerous situations and remain calm in an emergency.  Must be able to utilize behavior management techniques and serve as a role model.   Must be able to interact in a warm compassionate manner; de-escalating verbally and/or physically abusive and aggressive clients while utilizing aggression control techniques to ensure the safety and well-being of the client, other clients, and employees within the department.

Must demonstrate strong qualities of maturity, patience, good communication, self-awareness, and problem-solving skills. Must be comfortable working in a structured environment with established treatment models. Must successfully complete agency, state, and regulatory required training. Must be able to maintain professional boundaries and agency protocols for confidentiality.  Must be able to communicate effectively with suicidal, mentally, emotionally, and/or psychologically disturbed clients in person and over the telephone.
