POSITION:    Community Support Associate

WORKSITE:   Green County Clinic

DEPT:             Clinical Services

DEGREE:         HS/GED with 1 year experience working with individuals who receives services for primary                           mental health disorder and one of these co-occurring is orders; substance abuse, IDD, or                             physical health disorder.

TYPE:               Regular Full-Time

SKILLS:             Works directly with case managers and assists in the delivery of case management                                      services, monitor and coordinate consumer’s progress and provide transport to clients to                            required appointments.
