POSITION TITLE: Community Support Associate

WORKSITE: Pulaski Clinic/Somerset, KY

DEPARTMENT: Clinical Services

DEGREE/CREDENTIAL: High School/GED with 1 year experience working with individuals who receive services for primary mental health disorders and one of these co-occurring is orders; substance abuse, IDD, or physical health disorder.

TYPE: Regular Full-time

SKILLS: Works directly with case managers and assists in the delivery of case management services, monitors and coordinates consumer’s progress, and provides transport to clients to required appointments
