Principle Duties:

  • Assist in providing direct clinical services such as providing attendant care (self-care and self-help skills).
  • Monitor delivery of client services to insure compliance with treatment plans/IHP/ISP and consumer needs.
  • Assist in determining individual strengths and needs and provide observational feedback and participate in the Interdisciplinary Planning Team for the development of Individualized plans.
  • Develop individual and group training, activity schedules and curricula that assist in designing training plans with measurable goals and objectives that meets the individual needs, within and outside, the worksite.
  • Provide individualized/group instruction for individuals and integrate individual training objectives into daily routines while providing opportunities for incidental learning to meet the individual’s total needs.
  • Facilitate consumer’s integration in the community
  • Assess all environmental factors prior to any formal intervention to modify an individual’s behavior.
  • Provide supervision based on individual’s needs and assist as needed.
  • Administer health care services on an individual basis; administering medications, first aid and CPR, as well as follow through directives given by any health care provider.
  • As per agency policy and procedure keep current records and documentation on health care services provided such as medication logs, dietary restrictions, or behavioral logs.
  • May be required to use personal vehicle to transport clients.
  • Participate with process improvement activities as directed by program needs.
  • Report and document incidents (unusual or emergency) per agency policy and guidelines and be able to implement emergency procedures as needed.
  • Develop a working relationship with family and encourage participation in planning and implementation of services; provide additional support to individuals whom are affected by personal/familial situations or events.
  • Coordinate, monitor and document individual progress in programs records and the individual’s plan according to agency procedures.
  • Maintain a healthy environment that minimizes risk and facilitates providing proper care to the individual.
  • Provide all staff with the pertinent information pertaining to an individual’s plan, goals, intervention, and behaviors in order to coordinate maintain consistency in program services
